We are currently seeking an RN for the position of Clinical Care Coordinator.  This is a full time, Administrative position.  The Clinical Care Coordinator is responsible for coordinating the care of patients, and acts as a laiason between the field clinicians, physicians, patients and family members to ensure complete and quality services are provided.  Oversees documentation from field clinicians to ensure compliance with Medicare guidelines, state licensure requirements, and Joint Commission regulations.  Utilizes a computerized documentation system to record communication notes and review patient records.
